The 2025-2026 Activity Card 'Sticker'  - Number/Sticker for Student ID

Activity Cards and Final Forms Registration are a requirement for all students participating in athletics, newspaper, yearbook, student government, performing arts, and all other extracurricular clubs supported by Roosevelt ASB.

The ASB/Activity Fees are waived (FREE) for Free & Reduced Lunch (FRL) program participants - please bring Student ID to Fiscal Secretary for ASB sticker. 

Student ID's are distributed in October. If you pay for an activity card or are FRL, and do not get a printed ASB number or sticker on your Student ID, please bring your student ID to the fiscal secretary for the sticker.

Please note:  On and after September 20th, the yearbook and activity card may only be purchased separately .
